As you develop an action plan, please use the Narrative Form to record your information. This form (you must log in below to access it) allows you to enter information, save your work, and return later to complete it. When your action plan is complete, you can submit it to the Human Resources Department through this website.
The site also includes links to documents that you can download and share with others in your School/Area.
